What Is a Chain of Custody Form?
A Chain of Custody (COC) form accompanies samples submitted to the laboratory and documents sample collection details, identification, preservation, and requested testing. COC records support sample traceability and provide the documentation needed to support regulatory, legal, quality assurance, accreditation, and certification requirements.

Who Uses a Chain of Custody Form
Any party collecting, transporting, or receiving samples for analytical testing uses a chain of custody form. The COC is the official record that documents sample information, requested testing, and custody requirements, helping ensure accurate communication and traceability from sample collection through laboratory receipt.

Key information includes sample identification, collection location, collection date and time, collector name and signature, preservation details, requested analyses, and any required custody transfer documentation.
Missing information can delay sample login and review. Since holding times are calculated from the time of collection, documentation issues may impact the laboratory’s ability to complete testing within required holding times.
